What is a Health Care Advance Directive?
It's a single Florida document that combines three things: a Living Will (your wishes about life-prolonging procedures), a Health Care Surrogate Designation (the person who makes medical decisions for you), and a Health Care Power of Attorney. It draws its authority from Florida Statutes Chapters 765 and 709.
What does a Health Care Surrogate do?
Your surrogate is the person you trust to make medical decisions for you if you're unable to make them yourself. You also name an alternate surrogate who steps in if your primary surrogate is unavailable. By default their authority begins when a physician determines you can't decide — but you can choose to let them receive information or act sooner.
Do I need witnesses?
Yes. A Florida Health Care Advance Directive must be signed in the presence of two witnesses. At least one witness may not be your spouse or a blood relative, and your surrogate cannot serve as a witness. The initial, witness, and notary lines are left blank in your download so they can be completed in person.
Does this need to be notarized?

Can I change or revoke it later?
